I, the parent or legal guardian of the named applicant, a minor who is attending the Louisiana Royal Rangers Leadership Training , do give consent beforehand (in the event that all reasonable attempts to contact me have been unsuccessful) for the administration of any treatment necessary by a licensed physician or dentist. Also, By digitally signing this document I acknowledge that my child's likeness may be used for video, photographs, internet media, social media posts, etc.
